ACQUAWORLD REGISTRATION FORM

READ CAREFULLY BEFORE SIGNING

 

In consideration of being allowed to participate in any activities at Acqua World Swim Academy, including but not limited to swimming lessons, camps, free swim, and any other water-related activities (the "Activities"), I, the undersigned, acknowledge, appreciate, and agree to the following:

 

1. Assumption of Risk

I understand and acknowledge that participation in swimming and water-related activities carries inherent risks, including but not limited to drowning, slipping, falling, injuries from collisions, exposure to infectious diseases, and other unforeseen hazards. I further acknowledge that these risks could result in serious injury, permanent disability, or death. I voluntarily assume full responsibility for any risks, known or unknown, arising from my participation or my child's participation in the Activities.

 

2. Waiver and Release of Liability

I, on behalf of myself, my child, my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS Acqua World Swim Academy, its owners, directors, officers, employees, instructors, agents, volunteers, sponsors, and any affiliated organizations*("Releasees") from and against any and all claims, liabilities, damages, or causes of action, whether arising from negligence or otherwise, related to any injury, disability, death, illness, loss, or damage to person or property that may occur as a result of participation in the Activities.

3. Medical Authorization

In the event of an emergency, I authorize Acqua World Swim Academy and its staff to obtain medical treatment for me or my child at my expense. I understand that Acqua World Swim Academy does not provide medical insurance, and any costs incurred due to medical treatment will be my sole responsibility.

4. Compliance with Rules and Supervision

I agree to abide by all rules, regulations, and safety instructions set forth by Acqua World Swim Academy. I understand that failure to comply may result in immediate removal from the program without a refund. I further acknowledge that while lifeguards and instructors may be present, constant supervision is not guaranteed, and personal responsibility is required at all times.

5. Photography and Media Release

I grant permission to Acqua World Swim Academy to take photographs or videos of me or my child for promotional or educational purposes. I understand that these materials may be used on social media, websites, or other marketing materials without compensation.

6. Acknowledgment and Signature

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. I UNDERSTAND THAT BY SIGNING THIS DOCUMENT, I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.
